What is physical therapy?

 

Physical therapists are experts in the science and study of human movement. When one area of the body is out of sorts, it causes a chain reaction of compensation patterns up and down the body. It is the job of a physical therapist to discover the underlying cause of pain and dysfunction, and then prescribe the right techniques and exercises for healing.

 

Do you accept my insurance? 

 

Elevate is an out-of-network provider. Being out-of-network allows us to work with you one-on-one for a full hour without limitations dictated by insurance companies. 

We can issue superbills to you upon your request to submit for reimbursement. The superbill is a detailed invoice that itemizes the physical therapy services you received, which will include the necessary information to create a healthcare claim. 

Most insurance carriers allow you to submit claims via fax, mail, or online through a patient portal. We recommend you contact your healthcare plan’s member services directly for instruction on how to submit claims, for any policy requirements, and potential reimbursement prior to receiving treatment. 


Do you take Medicare?

 

At the current time, Medicare laws restrict us from providing physical therapy in a private pay model. Our therapists who are certified in Pilates are able to provide 1:1 private sessions, integrating the principals of optimal spinal mechanics, muscle engagement, and safe stretching specific to each person’s unique needs and presentation, but please note that these services are not eligible for reimbursement if you are a Medicare beneficiary at any level.


Can I use my Flex Spending Account (FSA) or Health Savings Account (HSA) to pay for services?

 

Yes, you can use your FSA or HSA to take care of payment for physical therapy sessions. 


What is your cancellation policy?

 

Elevate has a 24-hour cancelation policy, which requires at least one full day’s notice ahead of any booked appointment to cancel or reschedule the session. Any cancellations within the 24-hour cancellation time period will incur a late fee equal to the full session rate. 

 

Do I need a prescription to begin physical therapy?

 

New York and Florida are “direct access” states which mean that you can seek out physical therapy services without a doctor’s prescription. However, if treatment spans more than 30 days or 10 treatment sessions (whichever comes first), then a doctor’s prescription will be required to continue.


Do you make house calls?

 

We make house calls on a case-by-case basis.
